Credit/Debit Card number required to hold appointment. The card will not be billed until your appointment. 24 hour appointment cancellation is required, or you will be charged a non-cancellation/no-show fee of $50.00. (Check, cash, and money order also accepted to hold appointment.)
We do not directly bill your medical insurance company, but can provide you with documents to submit to your insurance company for possible reimbursement.
We accept VISA/Mastercard, debit cards, cash and bank checks.
All pharmacy items are non-returnable, regardless of whether they have been opened.
